Vzhledem k tomu, \u017ee posl\u00e1n\u00ed m\u00e9 laborato\u0159e se zam\u011b\u0159uje na prevenci rakoviny, nemohu s \u010dist\u00fdm sv\u011bdom\u00edm tuto nejasnost ignorovat.<\/span><\/p>\n<p><span dir=\"auto\">Jak jsme s m\u00fdm kolegou, mezin\u00e1rodn\u011b uzn\u00e1van\u00fdm onkologick\u00fdm biologem Dr. Wafikem El-Deirym, nast\u00ednili na z\u00e1\u0159ijov\u00e9m setk\u00e1n\u00ed ACIP o vakc\u00edn\u00e1ch proti COVID-19, t\u00e9m\u011b\u0159 50 publikac\u00ed uv\u00e1d\u00ed \u010dasovou souvislost mezi o\u010dkov\u00e1n\u00edm mRNA COVID-19 a v\u00fdskytem rakoviny. Epidemiologick\u00e9 studie (jedna z It\u00e1lie a jedna z Ji\u017en\u00ed Koreje) tak\u00e9 popsaly zv\u00fd\u0161en\u00fd v\u00fdskyt rakoviny u o\u010dkovan\u00fdch jedinc\u016f ve srovn\u00e1n\u00ed s neo\u010dkovan\u00fdmi jedinci (i kdy\u017e s v\u00fdhradami). T\u011bchto zpr\u00e1v p\u0159ib\u00fdv\u00e1 a je na\u010dase uznat, \u017ee se m\u016f\u017ee d\u00edt n\u011bco v\u00fdznamn\u00e9ho, \u200b\u200bsp\u00ed\u0161e ne\u017e je rovnou odm\u00edtat; ta druh\u00e1 se zd\u00e1 b\u00fdt p\u0159evl\u00e1daj\u00edc\u00ed reakc\u00ed ve v\u011bd\u011b, m\u00e9di\u00edch a na\u0161ich regula\u010dn\u00edch org\u00e1nech.<\/span><\/p>\n<p><span dir=\"auto\">M\u00fdm c\u00edlem je vysv\u011btlit v\u011bdeck\u00e9 poznatky a demonstrovat v\u011brohodn\u00e9 biologick\u00e9 mechanismy souvislosti mezi mRNA vakc\u00ednou proti Covidu a rakovinou, a od\u016fvodnit tak dal\u0161\u00ed a nal\u00e9hav\u00fd v\u00fdzkum. \u00da\u010delem nen\u00ed \u010dinit tvrzen\u00ed, ale nast\u00ednit probl\u00e9m, kter\u00fd je t\u0159eba \u0159e\u0161it, v nad\u011bji, \u017ee otev\u0159en\u00e1 v\u011bdeck\u00e1 diskuse a p\u0159edev\u0161\u00edm financov\u00e1n\u00ed v\u00fdzkumu bude mo\u017en\u00e9 zam\u011b\u0159it na tuto nal\u00e9havou a rostouc\u00ed oblast z\u00e1jmu. Sou\u010dasn\u00e1 situace v\u011bdc\u016fm znemo\u017enila zkoumat tuto problematiku bez obav z osobn\u00edch nebo profesn\u00edch n\u00e1sledk\u016f.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Co v\u00edme a co nev\u00edme<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">V sou\u010dasn\u00e9 dob\u011b neexistuj\u00ed \u017e\u00e1dn\u00e9 publikovan\u00e9 studie, kter\u00e9 by prokazovaly p\u0159\u00edm\u00fd kauz\u00e1ln\u00ed mechanismus, kter\u00fdm mRNA vakc\u00edny zp\u016fsobuj\u00ed rakovinu. To v\u0161ak neznamen\u00e1, \u017ee takov\u00e1 kauz\u00e1ln\u00ed souvislost neexistuje. Ve skute\u010dnosti existuj\u00ed nejm\u00e9n\u011b t\u0159i biologicky pravd\u011bpodobn\u00e9 mechanismy, kter\u00e9 si podle m\u00e9ho n\u00e1zoru zaslou\u017e\u00ed d\u016fkladn\u00e9 studium a vyhodnocen\u00ed vzhledem k jejich zn\u00e1m\u00e9 souvislosti s rozvojem rakoviny. O t\u011bchto mechanismech jsem psal v jin\u00fdch kontextech, ale zde vysv\u011btl\u00edm, jak by se mohly vztahovat na mRNA vakc\u00edny proti COVID-19.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Mechanismus 1: Transformace bun\u011bk prost\u0159ednictv\u00edm biologie spike proteinu<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">Transformace norm\u00e1ln\u00ed bu\u0148ky na rakovinnou bu\u0148ku zahrnuje naru\u0161en\u00ed n\u011bkolika ochrann\u00fdch mechanism\u016f, kter\u00e9 \u0159\u00edd\u00ed r\u016fst bun\u011bk, jejich p\u0159e\u017eit\u00ed a opravu DNA. mRNA vakc\u00edny proti COVID-19 funguj\u00ed tak, \u017ee instruuj\u00ed bu\u0148ky t\u011bla, aby po del\u0161\u00ed dobu (od dn\u016f a t\u00fddn\u016f a\u017e po m\u011bs\u00edce a dokonce i roky) produkovaly spike protein SARS-CoV-2. Tento ciz\u00ed spike protein pak spust\u00ed imunitn\u00ed odpov\u011b\u010f.<\/span><\/p>\n<p><span dir=\"auto\">Laboratorn\u00ed studie uk\u00e1zaly, \u017ee spike protein, bez ohledu na to, zda je produkov\u00e1n infekc\u00ed nebo o\u010dkov\u00e1n\u00edm, vykazuje biologickou aktivitu. Interaguje s bun\u011b\u010dn\u00fdmi dr\u00e1hami, kter\u00e9 reguluj\u00ed bun\u011b\u010dn\u00fd cyklus, funkce supresor\u016f n\u00e1dor\u016f a dr\u00e1hy a mechanismy opravy DNA. Takov\u00e9 interakce spike proteinu s t\u011bmito dr\u00e1hami by proto teoreticky mohly p\u0159isp\u00edvat k bun\u011b\u010dn\u00e9 transformaci \u2013 a\u010dkoli tot\u00e9\u017e by se dalo \u0159\u00edci o samotn\u00e9 infekci COVID-19. Rozd\u00edl v\u0161ak spo\u010d\u00edv\u00e1 v d\u00e9lce produkce spike proteinu po o\u010dkov\u00e1n\u00ed ve srovn\u00e1n\u00ed s p\u0159irozenou infekc\u00ed. To tak\u00e9 vyvol\u00e1v\u00e1 d\u016fle\u017eitou ot\u00e1zku: jsou opakovan\u00e9 infekce COVID biologicky ekvivalentn\u00ed um\u011bl\u00e9mu spike proteinu produkovan\u00e9mu vakc\u00ednou?<\/span><\/p>\n<p><span dir=\"auto\">Vzhledem k tomu, \u017ee spike protein produkovan\u00fd mRNA m\u016f\u017ee p\u0159etrv\u00e1vat n\u011bkolik dn\u00ed a\u017e po t\u00fddny, m\u011bs\u00edce a dokonce i roky po o\u010dkov\u00e1n\u00ed, je d\u016fle\u017eit\u00e9 rozpoznat, zda v\u00fdskyt rakoviny koreluje s expres\u00ed (nebo perzistenc\u00ed) spike proteinu v t\u011ble a tak\u00e9 zda je p\u0159\u00edtomen v n\u00e1dorech. Ned\u00e1vn\u00e1 p\u0159\u00edpadov\u00e1 studie poskytla d\u016fkazy o tom, \u017ee spike protein lze nal\u00e9zt exprimovan\u00fd v metastatick\u00e9m karcinomu prsu. Proto je p\u0159i zva\u017eov\u00e1n\u00ed vztahu mezi o\u010dkov\u00e1n\u00edm proti COVID-19 a rakovinou z\u00e1sadn\u00ed vz\u00edt v \u00favahu chronickou expozici l\u00e1tce s biologickou aktivitou, kter\u00e1 naru\u0161uje bun\u011b\u010dn\u00fd cyklus a dr\u00e1hy reakce na po\u0161kozen\u00ed DNA. Zcela odm\u00edtnout tuto skute\u010dnost by bylo nedbalostn\u00ed. V sou\u010dasn\u00e9 dob\u011b nejsou k dispozici dostate\u010dn\u00e1 data k vyvozen\u00ed jak\u00fdchkoli pevn\u00fdch z\u00e1v\u011br\u016f, a p\u0159i absenci takov\u00fdch \u00fadaj\u016f to znamen\u00e1, \u017ee tento mechanismus nelze zcela odm\u00edtnout.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Mechanismus 2: Genomick\u00e1 integrace a dysregulovan\u00e1 genov\u00e1 exprese v d\u016fsledku kontaminant\u016f zbytky DNA<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">V\u00fdrobci, FDA a dal\u0161\u00ed, v\u010detn\u011b laborato\u0159e NIH, nyn\u00ed uzn\u00e1vaj\u00ed, \u017ee v mRNA vakc\u00edn\u00e1ch je p\u0159\u00edtomna zbytkov\u00e1 kontaminace DNA.<\/span><\/p>\n<p><span dir=\"auto\">A\u010dkoli mnoz\u00ed argumentovali, \u017ee mno\u017estv\u00ed p\u0159\u00edtomn\u00fdch ve vakc\u00edn\u00e1ch je p\u0159\u00edli\u0161 mal\u00e9 na to, aby zp\u016fsobilo \u0161kodu, fakta z\u016fst\u00e1vaj\u00ed platn\u00e1: (1) tyto fragmenty existuj\u00ed, (2) jsou pod\u00e1v\u00e1ny v lipidov\u00e9 nano\u010d\u00e1stici, kter\u00e1 \u00fa\u010dinn\u011b umo\u017e\u0148uje DNA vstup do bun\u011bk a j\u00e1dra, a (3) velikost t\u011bchto fragment\u016f se m\u016f\u017ee snadno integrovat do genomu \u2013 zejm\u00e9na kdy\u017e se bu\u0148ky d\u011bl\u00ed a podl\u00e9haj\u00ed p\u0159irozen\u00e9 oprav\u011b DNA. Vzhledem k tomu, \u017ee nebyly provedeny \u017e\u00e1dn\u00e9 studie, kter\u00e9 by prok\u00e1zaly, \u017ee mno\u017estv\u00ed t\u011bchto ne\u010distot je nedostate\u010dn\u00e9 pro transfekci bun\u011bk a \u017ee se neintegruj\u00ed, je v sou\u010dasn\u00e9 dob\u011b pouhou spekulac\u00ed, \u017ee se to nem\u016f\u017ee a ani nestane. Jin\u00fdmi slovy, dosud \u017e\u00e1dn\u00e9 studie neprok\u00e1zaly, \u017ee tyto ne\u010distoty jsou p\u0159\u00edli\u0161 minim\u00e1ln\u00ed na to, aby vstoupily do bun\u011bk nebo se integrovaly do DNA.<\/span><\/p>\n<p><span dir=\"auto\">U vakc\u00edny Pfizer n\u011bkter\u00e9 ne\u010distoty obsahuj\u00ed sekvence DNA, kter\u00e9 jsou virov\u00fdmi regula\u010dn\u00edmi prvky, je\u017e ze sv\u00e9 podstaty ovliv\u0148uj\u00ed genovou expresi. Nov\u00e9 d\u016fkazy nav\u00edc nazna\u010duj\u00ed, \u017ee vakc\u00edna Pfizer obsahuje tak\u00e9 methylovanou DNA, kter\u00e1 m\u016f\u017ee stimulovat dr\u00e1hu v bu\u0148k\u00e1ch zvanou cGAS-STING. Tyto ne\u010distoty DNA se tedy, alespo\u0148 v p\u0159\u00edpad\u011b vakc\u00edny Pfizer, mohou nejen integrovat, ale mohou m\u00edt potenci\u00e1ln\u011b dalekos\u00e1hl\u00e9 \u00fa\u010dinky.<\/span><\/p>\n<p><span dir=\"auto\">Ud\u00e1losti integrace DNA v nespr\u00e1vn\u00e9m genomov\u00e9m kontextu by v principu mohly naru\u0161it genovou expresi a p\u0159isp\u011bt k bun\u011b\u010dn\u00e9 transformaci, zejm\u00e9na v kombinaci s prodlou\u017eenou aktivac\u00ed dr\u00e1hy cGAS-STING a regulac\u00ed promotoru genu SV40.<\/span><\/p>\n<p><span dir=\"auto\">Z\u00e1kladn\u00edm kamenem molekul\u00e1rn\u00ed biologie je schopnost pou\u017e\u00edvat lipidov\u00e9 nano\u010d\u00e1stice k doru\u010dov\u00e1n\u00ed DNA do bun\u011bk. Nepopirateln\u00fdm vedlej\u0161\u00edm \u00fa\u010dinkem je, \u017ee se \u010d\u00e1st DNA integruje. A kdy\u017e se integruje, m\u00e1 schopnost zm\u011bnit genovou expresi a naru\u0161it genovou funkci. P\u0159edpokl\u00e1dat, \u017ee se to nem\u016f\u017ee st\u00e1t s DNA kontaminanty v mRNA vakc\u00edn\u00e1ch, je zav\u00e1d\u011bj\u00edc\u00ed. Prost\u011b nev\u00edme, co se stane s DNA kontaminanty v mRNA vakc\u00ednov\u00fdch produktech, kdy\u017e p\u0159ijdou do kontaktu s bu\u0148kami (a\u0165 u\u017e in vitro nebo in vivo). Neexistuj\u00ed \u017e\u00e1dn\u00e9 \u00fadaje, kter\u00e9 by tvrdily, \u017ee se to nem\u016f\u017ee st\u00e1t a \u017ee se to ned\u011bje ani po o\u010dkov\u00e1n\u00ed.<\/span><\/p>\n<p><span dir=\"auto\">T\u00e9m\u011b\u0159 v\u0161ichni molekul\u00e1rn\u00ed biologov\u00e9 se shoduj\u00ed, \u017ee zaveden\u00ed DNA v lipidov\u00fdch nano\u010d\u00e1stic\u00edch do bun\u011bk p\u0159edstavuje transfekci DNA \u2013 jednodu\u0161e a jasn\u011b. Prost\u0159ednictv\u00edm tohoto mechanismu (a \u00fa\u010dink\u016f integrace promotorov\u00e9 sekvence SV40 a transfekovan\u00e9 methylovan\u00e9 DNA) je teoreticky mo\u017en\u00e9, \u017ee za spr\u00e1vn\u00e9ho kontextu by kontaminanty DNA mohly spustit nebo \u0159\u00eddit bun\u011b\u010dnou transformaci. Otev\u0159enou ot\u00e1zkou je, jak \u010dasto k tomu doch\u00e1z\u00ed a zda v\u016fbec k tomu doch\u00e1z\u00ed. Zat\u00edm nen\u00ed odpov\u011b\u010f zn\u00e1ma a jak ji\u017e bylo zm\u00edn\u011bno, nikdo nezkoum\u00e1, zda a jak \u010dasto k tomu doch\u00e1z\u00ed. Proto v tuto chv\u00edli nem\u016f\u017eeme vyvodit \u017e\u00e1dn\u00e9 z\u00e1v\u011bry pro ani proti t\u011bmto mechanism\u016fm.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Mechanismus 3: Imunitn\u00ed dysregulace: Nejpravd\u011bpodobn\u011bj\u0161\u00ed spojen\u00ed<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">Nejpravd\u011bpodobn\u011bj\u0161\u00ed mechanismus spojuj\u00edc\u00ed o\u010dkov\u00e1n\u00ed s rakovinou, zejm\u00e9na pokud jde o \u010dasov\u00e9 souvislosti, zahrnuje imunitn\u00ed syst\u00e9m. N\u011bkolik recenzovan\u00fdch studi\u00ed dokumentovalo imunitn\u00ed zm\u011bny po opakovan\u00e9 mRNA vakcinaci, v\u010detn\u011b zv\u00fd\u0161en\u00fdch z\u00e1n\u011btliv\u00fdch cytokin\u016f, vy\u010derp\u00e1n\u00ed T-bun\u011bk, zv\u00fd\u0161en\u00e9 produkce protil\u00e1tek IgG4 a p\u0159echodn\u00e9 imunosuprese.<\/span><\/p>\n<p><span dir=\"auto\">Imunitn\u00ed syst\u00e9m funguje jako kl\u00ed\u010dov\u00fd str\u00e1\u017ece br\u00e1ny proti rakovin\u011b, identifikuje a eliminuje transformovan\u00e9 bu\u0148ky d\u0159\u00edve, ne\u017e se mohou \u0161\u00ed\u0159it. M\u016f\u017ee tak\u00e9 p\u016fsobit jako siln\u00fd karcinogen a hnac\u00ed s\u00edla rakoviny ve form\u011b z\u00e1n\u011btu, zejm\u00e9na pokud je chronick\u00fd. Pokud je tedy imunitn\u00ed syst\u00e9m do\u010dasn\u011b naru\u0161en\u00fd, dysregulovan\u00fd nebo nadm\u011brn\u011b reaktivn\u00ed, m\u016f\u017ee kombinace selh\u00e1n\u00ed imunitn\u00edho dohledu a chronick\u00e9ho z\u00e1n\u011btu nejen zp\u016fsobit proliferaci ji\u017e existuj\u00edc\u00edch abnorm\u00e1ln\u00edch bun\u011bk, ale ve skute\u010dnosti je podpo\u0159it sm\u011brem k \u00fapln\u00e9 neoplastick\u00e9 transformaci. To by mohlo v\u00e9st k podpo\u0159en\u00e9 a dokonce urychlen\u00e9 tumorigenezi, kter\u00e1 je snadno pozorovateln\u00e1 v popsan\u00fdch \u010dasov\u00fdch r\u00e1mc\u00edch.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Na\u010dasov\u00e1n\u00ed a v\u00fdvoj rakoviny<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">V\u00fdvoj v\u011bt\u0161iny solidn\u00edch n\u00e1dor\u016f trv\u00e1 roky. Proto je nepravd\u011bpodobn\u00e9, \u017ee by se rakovina, kter\u00e1 se vyvine b\u011bhem 6\u201312 m\u011bs\u00edc\u016f po o\u010dkov\u00e1n\u00ed (s v\u00fdjimkou n\u011bkter\u00fdch lymfom\u016f, kter\u00e9 se mohou z po\u010d\u00e1te\u010dn\u00ed malign\u00ed transformace vyvinout b\u011bhem n\u011bkolika t\u00fddn\u016f a\u017e n\u011bkolika m\u011bs\u00edc\u016f), vyvinula v d\u016fsledku spou\u0161t\u011bc\u00edch ud\u00e1lost\u00ed zp\u016fsoben\u00fdch mRNA vakc\u00ednou prost\u0159ednictv\u00edm mechanismu 1 nebo 2.<\/span><\/p>\n<p><span dir=\"auto\">I kdy\u017e v\u0161ak mRNA vakc\u00edna proti COVID-19 nen\u00ed spou\u0161t\u011bc\u00edm faktorem, z\u016fst\u00e1vaj\u00ed pravd\u011bpodobn\u00e9 sc\u00e9n\u00e1\u0159e, ve kter\u00fdch by preexistuj\u00edc\u00ed premalign\u00ed nebo okultn\u00ed rakovinn\u00e9 bu\u0148ky (ji\u017e geneticky nestabiln\u00ed a p\u0159ipraven\u00e9 k \u00fapln\u00e9 neoplastick\u00e9 transformaci) mohly b\u00fdt urychleny nezam\u00fd\u0161len\u00fdmi \u00fa\u010dinky spike proteinu nebo vz\u00e1cn\u00fdmi ud\u00e1lostmi integrace DNA. Krom\u011b toho by jak\u00e1koli sp\u00edc\u00ed nebo mikroskopick\u00e1 rakovina dr\u017een\u00e1 pod kontrolou imunitn\u00edm dohledem mohla b\u00fdt v principu uvoln\u011bna nebo podporov\u00e1na imunitn\u00ed dysregulac\u00ed (mechanismus 3).<\/span><\/p>\n<h3><strong><span dir=\"auto\">Vzory, kter\u00e9 lze pozorovat<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">N\u011bkolik studi\u00ed dokumentovalo m\u011b\u0159iteln\u00e9 zm\u011bny v imunitn\u00edch funkc\u00edch po opakovan\u00e9 vakcinaci mRNA, v\u010detn\u011b z\u00e1n\u011btu, autoimunity a formy z\u00edskan\u00e9 funk\u010dn\u00ed imunodeficience. Tyto zm\u011bny byly zdokumentov\u00e1ny i u dlouhodob\u011b prok\u00e1zan\u00fdch pacient\u016f s COVID-19; proto bude d\u016fle\u017eit\u00e9 analyzovat trendy a vzorce v datech mezi o\u010dkovan\u00fdmi a neo\u010dkovan\u00fdmi jedinci, stejn\u011b jako mezi o\u010dkovan\u00fdmi a neo\u010dkovan\u00fdmi pacienty s dlouhodob\u00fdm onemocn\u011bn\u00edm COVID-19.<\/span><\/p>\n<p><span dir=\"auto\">Vzhledem k tomu, \u017ee imunodeficience je \u010dasto doprov\u00e1zena chronick\u00fdm z\u00e1n\u011btem, maj\u00ed oboj\u00ed p\u0159\u00edm\u00e9 d\u016fsledky pro sledov\u00e1n\u00ed n\u00e1doru a jeho permisivitu. Existuj\u00ed tedy sign\u00e1ly, kter\u00e9 by se daly o\u010dek\u00e1vat na z\u00e1klad\u011b p\u0159edv\u00eddateln\u00fdch vzorc\u016f rakoviny pozorovan\u00fdch u jin\u00fdch forem z\u00edskan\u00e9 imunodeficience (nap\u0159. u p\u0159\u00edjemc\u016f HIV nebo transplantovan\u00fdch org\u00e1n\u016f). Mechanismy, kter\u00e9 tyto druhy rakoviny \u0159\u00edd\u00ed, jsou dob\u0159e zn\u00e1m\u00e9 a mezi onkologick\u00fdmi biology \u0161iroce uzn\u00e1van\u00e9.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Lymfoidn\u00ed rakoviny<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">Prvn\u00edm a nejbezprost\u0159edn\u011bj\u0161\u00edm pozorov\u00e1n\u00edm by byl n\u00e1r\u016fst lymfoidn\u00edch malignit, zejm\u00e9na non-Hodgkinov\u00fdch lymfom\u016f (NHL), T-bun\u011b\u010dn\u00fdch lymfom\u016f a agresivn\u00edch B-bun\u011b\u010dn\u00fdch lymfom\u016f, jako jsou Burkitt\u016fv lymfom nebo dif\u00fazn\u00ed velkobun\u011b\u010dn\u00e9 B-bun\u011b\u010dn\u00e9 lymfomy (DLBCL). Tyto druhy rakoviny jsou \u00fazce spjaty s mechanismy imunitn\u00ed kontroly a onkogenez\u00ed EBV. Za podm\u00ednek imunitn\u00edho stresu nebo vy\u010derp\u00e1n\u00ed by B-bu\u0148ky s latentn\u00ed EBV infekc\u00ed mohly uniknout kontrole, podl\u00e9hat klon\u00e1ln\u00ed expanzi a z\u00edskat dal\u0161\u00ed genomov\u00e9 zm\u011bny nezbytn\u00e9 pro \u00faplnou transformaci.<\/span><\/p>\n<p><span dir=\"auto\">U imunokompromitovan\u00fdch pacient\u016f se takov\u00e9 lymfomy \u010dasto objevuj\u00ed b\u011bhem n\u011bkolika m\u011bs\u00edc\u016f od imunitn\u00ed dysfunkce. Podobn\u00e1 \u010dasov\u00e1 dynamika po opakovan\u00e9 vakcinaci mRNA nebo jak\u00e9koli trval\u00e9 imunitn\u00ed poru\u0161e by proto vy\u017eadovala pe\u010dliv\u00e9 epidemiologick\u00e9 zkoum\u00e1n\u00ed.<\/span><\/p>\n<p><span dir=\"auto\">Je pozoruhodn\u00e9, \u017ee v publikovan\u00fdch kazuistik\u00e1ch byl ne\u00fam\u011brn\u011b vysok\u00fd v\u00fdskyt postvakcina\u010dn\u00edch lymfom\u016f, a to jak nov\u011b vznikl\u00fdch p\u0159\u00edpad\u016f, tak i rychl\u00fdch relaps\u016f po remisi. Zda tato pozorov\u00e1n\u00ed p\u0159edstavuj\u00ed n\u00e1hodu, zkreslen\u00ed v hl\u00e1\u0161en\u00edch nebo skute\u010dnou imunitn\u00ed dysfunkci, z\u016fst\u00e1v\u00e1 nezn\u00e1m\u00e9. Samotn\u00fd vzorec je v\u0161ak biologicky konzistentn\u00ed s t\u00edm, co bychom o\u010dek\u00e1vali v p\u0159\u00edpad\u011b selh\u00e1n\u00ed imunitn\u00edho dohledu.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Rakoviny spojen\u00e9 s viry<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">Dal\u0161\u00ed kategori\u00ed rakoviny, u kter\u00e9 se o\u010dek\u00e1v\u00e1 n\u00e1r\u016fst, jsou n\u00e1dory virov\u00e9 etiologie, proto\u017ee jejich v\u00fdskyt je \u010dasto zp\u016fsoben selh\u00e1n\u00edm imunitn\u00edho dohledu. Pat\u0159\u00ed mezi n\u011b Kaposiho sarkom, karcinom z Merkelov\u00fdch bun\u011bk, cervik\u00e1ln\u00ed a orofarynge\u00e1ln\u00ed karcinom (vyvolan\u00fd HPV) a hepatocelul\u00e1rn\u00ed karcinom (HBV\/HCV). Tyto n\u00e1dory obvykle vznikaj\u00ed v souvislosti s imunosupres\u00ed, chronick\u00fdm z\u00e1n\u011btem nebo oboj\u00edm.<\/span><\/p>\n<p><span dir=\"auto\">N\u00e1r\u016fst t\u011bchto druh\u016f rakoviny, zejm\u00e9na u jedinc\u016f bez klasick\u00e9 imunosuprese, by mohl nazna\u010dovat naru\u0161en\u00ed imunitn\u00ed editace, co\u017e by vedlo ke ztr\u00e1t\u011b rovnov\u00e1hy mezi hostitelem a virem. Pokles imunitn\u00ed kontroly nad latentn\u00edmi HPV infekcemi by mohl urychlit onkogenn\u00ed progresi v d\u011blo\u017en\u00edm \u010d\u00edpku nebo orofaryngu. Podobn\u011b by sn\u00ed\u017een\u00e1 aktivita cytotoxick\u00fdch T-bun\u011bk mohla v\u00e9st k manifestaci subklinick\u00fdch Merkelov\u00fdch bun\u011b\u010dn\u00fdch nebo Kaposiho l\u00e9z\u00ed.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Leuk\u00e9mie a myelodysplastick\u00e9 syndromy<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">N\u011bkolik studi\u00ed \u010dasov\u00e9 asociace popsalo p\u0159\u00edpady akutn\u00ed leuk\u00e9mie a myelodysplastick\u00e9ho syndromu (MDS) po o\u010dkov\u00e1n\u00ed. Tyto malignity jsou vysoce citliv\u00e9 na z\u00e1n\u011btliv\u00e9 a imunomodula\u010dn\u00ed prost\u0159ed\u00ed, stejn\u011b jako na vlivy prost\u0159ed\u00ed, kter\u00e9 ovliv\u0148uj\u00ed integritu DNA. Proto je pravd\u011bpodobn\u00e9, \u017ee zv\u00fd\u0161en\u00ed trval\u00e9 imunitn\u00ed aktivace n\u00e1sledovan\u00e9 supres\u00ed by mohlo urychlit expanzi preleukemick\u00fdch klon\u016f, kter\u00e9 jsou ji\u017e p\u0159\u00edtomny ve st\u00e1rnouc\u00ed kostn\u00ed d\u0159eni. Je tak\u00e9 pravd\u011bpodobn\u00e9, \u017ee kontaminanty DNA v mRNA vakc\u00edn\u00e1ch by se mohly p\u0159ednostn\u011b integrovat do hematopoetick\u00fdch progenitorov\u00fdch bun\u011bk, kter\u00e9 jsou obzvl\u00e1\u0161t\u011b citliv\u00e9 na genotoxick\u00fd stres. Integrace do zraniteln\u00fdch genomov\u00fdch oblast\u00ed t\u011bchto bun\u011bk by teoreticky mohla iniciovat leukemickou transformaci.<\/span><\/p>\n<p><span dir=\"auto\">A\u010dkoliv takov\u00e1 klon\u00e1ln\u00ed dynamika m\u016f\u017ee b\u00fdt na \u00farovni populace nen\u00e1padn\u00e1, lze ji detekovat pomoc\u00ed longitudin\u00e1ln\u00edch studi\u00ed, zejm\u00e9na p\u0159i stratifikaci podle v\u011bku, o\u010dkovac\u00ed historie a marker\u016f imunitn\u00ed aktivace.<\/span><\/p>\n<h3><strong><span dir=\"auto\">Agresivn\u00ed nebo neobvykl\u00e9 solidn\u00ed n\u00e1dory<\/span><\/strong><\/h3>\n<p><span dir=\"auto\">Kone\u010dn\u011b by se dalo o\u010dek\u00e1vat, \u017ee v t\u011bsn\u00e9 \u010dasov\u00e9 bl\u00edzkosti po o\u010dkov\u00e1n\u00ed mRNA se vyskytnou vz\u00e1cn\u00e9 nebo neobvykle agresivn\u00ed solidn\u00ed n\u00e1dory. Mezi n\u011b by mohly pat\u0159it vysoce malign\u00ed gliomy, karcinomy slinivky b\u0159i\u0161n\u00ed, rychle proliferuj\u00edc\u00ed sarkomy, rakovina prsu a dal\u0161\u00ed solidn\u00ed n\u00e1dory.<\/span><\/p>\n<p><span dir=\"auto\">Na popula\u010dn\u00ed \u00farovni by se souvislost mezi rakovinou a o\u010dkov\u00e1n\u00edm pravd\u011bpodobn\u011b projevila jako ne\u00fam\u011brn\u00fd n\u00e1r\u016fst hematologick\u00fdch n\u00e1dor\u016f (lymfomy, leuk\u00e9mie) a n\u00e1dor\u016f spojen\u00fdch s viry ve srovn\u00e1n\u00ed s v\u00fdchoz\u00edmi trendy. Lze tak\u00e9 o\u010dek\u00e1vat n\u00e1r\u016fst v\u00fdskytu n\u00e1dor\u016f s d\u0159\u00edv\u011bj\u0161\u00edm n\u00e1stupem nebo shluky rychle progreduj\u00edc\u00edch \u010di na l\u00e9\u010dbu rezistentn\u00edch n\u00e1dor\u016f v kr\u00e1tk\u00fdch intervalech po o\u010dkov\u00e1n\u00ed, pokud by p\u0159\u00ed\u010dinou byl chronick\u00fd z\u00e1n\u011bt nebo vy\u010derp\u00e1n\u00ed T-bun\u011bk. Dormantn\u00ed, okultn\u00ed, in situ karcinomy nebo mikrometast\u00e1zy by se mohly aktivovat, pokud je imunitn\u00ed dohled utlumen\u00fd nebo pokud z\u00e1n\u011btliv\u00e9 cytokiny zm\u011bn\u00ed strom\u00e1ln\u00ed mikroprost\u0159ed\u00ed. Ty by se mohly snadno projevit b\u011bhem 12\u201336 m\u011bs\u00edc\u016f po o\u010dkov\u00e1n\u00ed.<\/span><\/p>\n<p><span dir=\"auto\">I kdy\u017e \u017e\u00e1dn\u00fd z t\u011bchto vzorc\u016f neprokazuje kauzalitu, nem\u011bl by b\u00fdt takov\u00fd vzorec zavrhov\u00e1n jako pouh\u00e1 n\u00e1hoda. S rakovinou byly spojov\u00e1ny i jin\u00e9 faktory environment\u00e1ln\u00ed expozice, jako je tab\u00e1k, azbest a endokrinn\u00ed disruptory. Po\u010d\u00e1te\u010dn\u00ed varov\u00e1n\u00ed se setkala se skepticismem, ale v ka\u017ed\u00e9m z t\u011bchto p\u0159\u00edklad\u016f d\u016fkladn\u00e9 studie, pozorov\u00e1n\u00ed a experiment\u00e1ln\u00ed v\u00fdzkum prok\u00e1zaly jejich kauz\u00e1ln\u00ed vztah. Stejn\u00fd princip by m\u011bl b\u00fdt uplatn\u011bn i zde. V\u00fdzkumn\u00edci mus\u00ed m\u00edt mo\u017enost tyto anal\u00fdzy replikovat a roz\u0161i\u0159ovat, bez cenzury, osobn\u00edch nebo profesion\u00e1ln\u00edch odvetn\u00fdch opat\u0159en\u00ed.<\/span><\/p>\n<p><span dir=\"auto\">Vyhodnocen\u00ed a kvantifikace t\u011bchto potenci\u00e1ln\u00edch mechanism\u016f se mus\u00ed st\u00e1t prioritou v\u00fdzkumu, pokud chceme porozum\u011bt rostouc\u00edmu po\u010dtu zpr\u00e1v spojuj\u00edc\u00edch vznik rakoviny s o\u010dkov\u00e1n\u00edm proti Covid-19 a ur\u010dit, zda tyto souvislosti odr\u00e1\u017eej\u00ed skute\u010dn\u00e9 kauz\u00e1ln\u00ed vztahy.<\/span><\/p>\n<p><span dir=\"auto\">Dlouhodob\u00e9 popula\u010dn\u00ed studie budou nezbytn\u00e9 k ur\u010den\u00ed, zda se ur\u010dit\u00e9 druhy rakoviny, zejm\u00e9na vz\u00e1cn\u00e9 nebo agresivn\u00ed podtypy, vyskytuj\u00ed \u010dast\u011bji u o\u010dkovan\u00fdch jedinc\u016f ve srovn\u00e1n\u00ed s neo\u010dkovan\u00fdmi.
